Familial medical history

The familial medical history questionnaire includes items about illnesses occurring in the family, including the parents of the mother and father, siblings, and other children.

The disorders or diseases include thrombosis in arm or leg, pulmonary embolism, thyroid disease, epilepsy, diabetes, high blood pressure, heart attack, stroke, migraine, mental retardation, congenital abnormalities, the age of menopause, high blood pressure during pregnancy, and toxaemia.
