Children's Sleep Habits Questionnaire (CSHQ) - Abbreviated/Short version

The Children's Sleep Habit Questionnaire - Short Form (CSHQ-SF) assessess the largest categories of behaviorally-based sleep disorders and medically-based sleep disorders. The questionnaire consists of 8 subscales: not going to bed, falling asleep late, not enough sleep (sleep duration), fear of sleeping, staying awake at night, parasomnia (including sleepwalking, nightmares, etc.), abnormal breathing and sleepiness during the day.
