Child Health Questionnaire - Parent Form (CHQ-PF28)

The Child Health Questionnaire - Parent Form (CHQ-PF28) is a questionnaire designed to assess the physical and psychosocial well-being of children aged 5-18 years. It consists of 28 items covering 13 domains, selected from the Child Health Questionnaire - Child Form (CHQCF-87). The questionnaire is completed by the child's parent or primary caregiver and is used to evaluate the child's health-related quality of life. Items include scales on role functioning (physical and behavior), mental health, general health perceptions, family activities, and family cohesion.
